NEW CLIENT INFORMATION

Congratulations on taking this step!

For some, it can be a relief to make a call to begin counseling, while others may be nervous about meeting with a counselor. Sometimes counseling is a last ditch approach to a problem or being forced upon someone by another. No matter your situation, it takes courage to take this first step and we would like to encourage you to create the path that best meets your needs.

People enter therapy for many reasons, here are just a few others have stated:

My life is out of control and unmanageable...and everyone says I should be in therapy.
I am in a trauma or crisis...something unexpected has just happened, such as, a loss, divorce, rejection, a memory of abuse, or having been attacked.
My life has not turned out the way I had planned, dreamed, or thought it would...and I do not know why! I need answers to what I might be doing wrong???
My partner seems to keep saying that my behavior is not OK. Yet I am not sure what they are speaking (and they just might be right). So I need a safe unbiased place I can go to figure it out.
I want to be happier. I believe in the process of therapy and want to make my life better...yet I need a bit of help in doing so. Personal growth is of value.

We are a practice qualified and trained to treat adults, teens, and children. We offer individual therapy, couples therapy, family therapy, and group therapy. If you are unsure of your needs, please speak to your counselor at your first meeting to determine and customize your counseling to meet your desired needs.

If you are just considering counseling and would like to set up a free 20 minute session to discuss the possibility of beginning counseling, we have several clinicians available to discuss all of your questions with you.

Our team uses a variety of approaches to meet the needs of our clients. We are creative and skilled at many methods. We use action, drawing, writing, movement, grounding, and often talking is all that is needed. We have several clinicians with training and knowledge of the following methods: Cognitive therapy, EMDR, ETT, Bioenergetics, Psychodrama, Imago, sandtray therapy just to name a few...

When working with children, teens, or families, this creative approach can be very useful. It is often important to be able to have flexibility and creativity to help kids or teens open up and be willing to discuss topics that may feel overwhelming or scary for them to discuss.

We have sliding scale fees if there is a financial problem, so please ask about this possibility. We have the availability to see clients during the daytime, evenings, and weekends, depending on which clinician you are working with.

If you have made your appointment with a therapist who best meets your needs it will be necessary for you to fill out the following paperwork and bring to your primary appointment. If you do not have the ability to print at your home, please let us know and we can provide papers for you:

Adult Client: Adult Intake Form, Hippa Signature Release Form, Consent Form for anyone you would like us to be able to have dialogue with (consent may be revoked at anytime you wish).

Adolescent Client (ages 12-18): Adolescent Intake Form, Hippa Signature Release Form, Consent Form for anyone you would like us to be able to have dialogue with (consent may be revoked at anytime you wish), Consent for treatment of a minor: There is a new LPC rule stating that if there are any custody paper's on a minor all paperwork must be on file with the treating counselor. Therefore, if your child who is being seen is under ANY custody arrangement please bring all paperwork.
